How It Works
Answer a set of health questions, upload any health data available PDF or EHR — on any device, self-directed or guided by staff.
Results surface a risk tier, flag the specific signals behind it, and suggest a concrete next step. The patient sees a plain-language summary. The care team sees the full clinical detail.
With a single tap, results are shared with the care team, family, or referring clinician — with the patient's explicit consent. Risk Insights feeds directly into the Care Plan layer for action.

Both models were trained on structured and unstructured outpatient data from Mayo Clinic — one of the world's most comprehensive longitudinal clinical datasets. The models are owned by SeniorLife and improve as more longitudinal data accumulates.
